Payroll Service Agreement
Pacific Northwest Tax Service is hereby authorized to perform the following payroll services on behalf of ________________________________:

- Submit applications to IRS and Oregon which will authorize Pacific Northwest Tax Service to pay payroll taxes directly through the automated EFTPS payment systems.
- Submit payroll tax deposit information into IRS and Oregon EFTPS systems for each pay period where payroll deposits are required.
- Prepare federal Form 941 FICA and withholding reports on a quarterly basis
- Prepare federal Form 940 FUTA unemployment tax reports on an annual basis
- Prepare and file Forms W2 and W3 with the Social Security Administration on an annual basis
- Prepare and file Form OQ Oregon Combined Quarterly reports on a quarterly basis using Oregon’s “Otter” payroll reporting system
- Prepare and file WR Oregon Annual Withholding Tax Reconciliation Report.
- Prepare and file Oregon Report for new hiring.
I shall submit payroll requests to Pacific Northwest Tax Service by either of the following methods:

To ensure accurate payroll reporting, payroll requests should include the following information:
- Completed form W4 for each new employee hired
- Date pay period ends
- Date paycheck is to be issued
- Name of employee being paid
- Rate of pay that applies
- Type of pay that applies – such as
- Regular hours
- Overtime hours
- Tips
- Vacation
- Salary
- Overtime must be separately stated from regular payroll (Overtime is mandatory under state law for all time worked in a single week that exceeds 40 hours. Overtime may not be reduced by averaging, comp time or other methods.)

How Do I Begin My Payroll Service?
It is our pleasure to work as your payroll service. We require the following information to start your payroll:
- Your business' federal EIN Number (If you do not have an EIN, we can obtain one for you)
- Your business' Oregon Business Identification Number (BIN) (if you do not have an Oregon BIN, we can obtain one for you)
- If you are switching from another payroll service company, we will need your year-to-date payroll figures and payroll reports.

- Completed Form W4 for each current and new employee hired (Click here to download Form W4)
- Completed Form I9 for each current and new employee hired (Click here to download Form I9)

- If you want us to initiate your payroll deposits through EFTPS, we will need the following:
- Your business checking account number
- Bank's branch name and address
- A cancelled check from the bank account your company will issue payroll through
- If you want us to issue employee payroll through direct deposit, we will need your banking information and a cancelled check from each employee who will be receiving payroll through direct deposit.
